Staying healthy with continued care

GRAND RAPIDS, Mich. (WOOD) There’s a facility in West Michigan that provides great care for older adults, and now they’re expanding in a big way! Sunset Home Services and Waterford Place just broke ground on a 69,000 square foot expansion that will house a rehabilitation center, inpatient therapy, and assisted living. It is scheduled to open in 2015.

The comprehensive health care approach by the team at Sunset Home Services, Brookcrest  Rehab Unit, and the Waterford Place staff is illustrated in a big way by their Congestive Heart Failure Program. They worked together to develop a series of guidelines to help older adults stay safe and out of the hospital. It’s a great example of how this coordination of information and care can keep people healthier.
