It’s a packet with a plan of care for each individual who’s treated at a Sunset Communities facility or through their home care program. It contains specific instructions, precautions, nutrition, proper wound care, and signs or symptoms of complications.

When scheduling your surgery, tell your doctor and the hospital staff that you want Brookcrest or Sunset Home Services for rehab. You can even call Brookcrest to say you’d like to be on the priority list because you are planning on rehab there or request Sunset Home Services for rehab before your surgery.
